Job Description:

We are seeking an PRN Hospice Nurse to join our team in West End, Western Colorado. This role is ideal for a compassionate and dedicated nurse who values the opportunity to make a meaningful impact on the lives of patients and their families in a rural setting. As an PRN Hospice Nurse, you will play a crucial role in providing exceptional care to our limited patient base, offering flexibility to balance work with other commitments.

Key Responsibilities:

– Conduct assessments and develop care plans for hospice patients.

– Administer medications, provide wound care, and manage symptom control.

– Collaborate with the interdisciplinary hospice team to ensure comprehensive care.

– Provide emotional and psychological support to patients and their families.

– Assist with the coordination of hospice services based on census and patient need.

– Maintain accurate and detailed patient records.

Qualifications:

– Current Registered Nurse (RN) license in the state of Colorado.

– Previous experience in hospice or palliative care is preferred.

– Compassion, empathy, and the ability to communicate effectively with patients and their families.

– Willingness to work on an as-needed basis, with visits ranging from 1-3 per week.

– Reliable transportation to access rural areas in West End.

– Strong organizational and time-management skills.

 On-Call Hospice Nurse (Seven Days On, Seven Days Off)

Job Description: Touch of Care Hospice is seeking a compassionate On-Call Hospice Nurse dedicated to exceptional end-of-life care. Work in a seven-day rotational schedule, ensuring continuous support for patients and families during critical times.

Responsibilities:

  • Provide skilled nursing care, pain management, and symptom control during on-call shifts.
  • Assess patients, devise care plans, and implement interventions for enhanced comfort.
  • Collaborate with interdisciplinary teams to address holistic patient needs.
  • Offer emotional support and education, fostering a caring environment.
  • Maintain accurate documentation of patient care.

Qualifications:

  • Valid RN license and nursing degree.
  • Preferably experience in hospice or palliative care settings.
  • Proficient in pain management and symptom control.
  • Strong communication and decision-making skills.
  • Willingness to work weekends, holidays, and a seven-day on, seven-day off schedule.

Compensation:

  • $50 per weekday and $100 per weekend day for on-call availability.
  • Additional compensation of $36-$42 per hour for on-call duties (DOE).
